Many older adults and their families find themselves trying to navigate systems that are unfamiliar to them. As Aging Advisors with years of experience in care management, we are equipped to guide you through these systems, helping you to understand them, as well as provide support and advocacy. We approach each situation with empathy, considering the whole person and their family, so that we can provide support tailored to individual needs.

Medical Advocacy

We serve as a knowledgeable advocate during medical visits and care transitions, helping to communicate concerns, clarify treatment options, and ensure our client’s wishes and best interests are honored. Visit summaries are provided after each visit to maintain continuity.

Resource Coordination

From in-home support to community services and long-term care options, which may include downsizing and moving to a new home, we connect clients with trusted, appropriate resources and coordinate services seamlessly.

I cannot say enough good things or recommend mission transitions strongly enough. My mom recently moved to Philly from CT, and the lovely and incredibly capable women of Mission Transitions have made every step of this hard and huge journey better and easier. For starters, they came and helped us on move any day. My mom moved to a senior living community and we know many folks there, after they moved, had boxes in their apartments for months, making it much harder to get settled. By the end of my mom‘s move-in day, her apartment looked spectacular. Between the physical and emotional labor of it all, there is no way we could’ve done it without them. There was a lot of “stuff”and a lot of decisions and having their help and support and kindness throughout the long day, especially from people who have done this before , helped us make better decisions about where things should go and how to make it safe and accessible for my mom , and it made all the difference. It’s a hard transition, but at least by the end of the day when my mom looked around, she felt comfortable and surrounded by familiar things and the day went smoothly. We have also been using Mission Transition ‘s for care management. My mom has had to find all new doctors and given I work and have a family of my own and no specialty in this area, I don’t know how I would’ve had the time or been able to do this without them. They are kind, knowledgeable, and sensitive to the world of seniors, especially someone like my mom going through such a major transition. They are incredibly accessible and flexible and have moved things along quickly, which is reassuring for my mom , me and my brother and our whole family. They also help with the actual appointments, getting her there, and also making sure she and me and my brother know what’s being said (And since they have been in the field of geriatrics for ages, they understand it better than any of the rest of us would, so they help us b break down what the doctors are saying, And keep us all informed, almost in real time With prompt follow ups and write ups from the appointments, all of which is invaluable. My my brother and I are kept informed and able to make decisions without having to actually be at all the appointments. ) My mom is 82 and in independent living and quite capable of a lot and enjoying life in Philly . And there’s lots of help also given by her senior living community, but even still , care management and all the caretaking with the doctors is not some thing she could do alone or even with just my family‘s help, And all the time I remark on how lucky we are to have found the wonderful women of Mission Transitions.
