Our abbreviated consultation offers a supportive starting point for older adults and their families who have questions, concerns, or are unsure where to begin. This option provides personalized insight, trusted guidance, and actionable recommendations to help older adults and families navigate aging-related decisions with confidence.
Care Management
Our comprehensive assessment provides a foundation for informed decision-making and planning based on our client’s wishes. As circumstances evolve, we work with our clients to provide ongoing support, regular check-ins, and adjustments to the care plan to enhance safety, independence, and quality of life.
Healthcare Navigation
The healthcare system can be complex and overwhelming. We help clients and families navigate appointments, specialists, hospitalizations, and transitions of care – ensuring information is organized, questions are answered, and next steps are clear.
Medical Advocacy
We serve as a knowledgeable advocate during medical visits and care transitions, helping to communicate concerns, clarify treatment options, and ensure our client’s wishes and best interests are honored. Visit summaries are provided after each visit to maintain continuity.
Resource Coordination
From in-home support to community services and long-term care options, which may include downsizing and moving to a new home, we connect clients with trusted, appropriate resources and coordinate services seamlessly.
